Financial Responsibility of Dental Treatment
Payment is due at the time of service for all treatment rendered. Cash, Check, Visa, Mastercard and Discover are all acceptable forms of payment.
We are happy to file your claim of services with your insurance company and we only accept the co-payment at the time of service. We are sensitive to the fact that payment alternatives and/or financial arrangements for treatment might be necessary.
About Dental Insurance
WE’VE GOT YOU COVERED
If you have dental insurance coverage, please provide the name, address and phone number of your insurance company, as well as your policy and group numbers. We will happily verify your insurance benefits before your appointment. Please e-mail us a photo of the front and back of your insurance card along with the primary account holders birthday and we will provide you an estimate of your coverage. This enables us to serve you more effectively by providing an estimate of your coverage.
We are only able to give an estimate of what your insurance will cover for procedures rendered at our office. We will do our best to ensure the most coverage possible, however you are still ultimately responsible for paying the entire balance. If you would like an exact estimation of your costs prior to your appointment, we’re happy to mail a pretreatment plan to your insurance company. Insurance requires 2-4 weeks to process pretreatments and send an estimate of coverage.
